The national medical examiner system is a key component of the Department of Health and Social Care’s (DHSC’s) Death Certification Reform Programme for England and Wales.

It also forms part of the NHS Patient Safety Strategy and the NHS Long Term Plan in England, and is a key element of the quality and patient safety agenda in Wales, making an important contribution to the long-established mortality review programme.

Implementation of medical examiners began in England and Wales in 2019 with the appointment of the National Medical Examiner and recruitment of national and regional teams.

This report summarises progress made during 2022.
